You can call this number to:
  • Start a referral for In-Home Supportive Services (IHSS), Medi-Cal LTC, or ALW (Assisted Living Waiver)

  • Request a social worker assessment for LTC needs

  • Ask for help with benefits, caregiving support, housing, or placement

📝 What You Should Prepare When You Call, Have the following ready:
  • Senior’s full name, DOB, and Medi-Cal number (if enrolled)

  • Diagnosis or concerns (e.g., dementia, mobility issues)

  • Current care situation (living alone, with family, already in facility, etc.)

  • What services you’re requesting (IHSS, ALW, skilled nursing, RCFE placement, etc.)

🚨 Urgent LTC Cases

If the senior is in the hospital, rehab, or at risk of homelessness:

  • Ask the hospital discharge planner to contact the county’s Adult Protective Services (APS) or LTC social worker directly for urgent help.

  • Call APS directly: (800) 675-8437 in San Mateo County (same number, different intake unit).
